Eligibility, Enrolling & Making Changes

Good news! If you’re a regular Amy’s team member, working 24+ hours per week, you’re eligible for benefits!

The following dependents are also eligible:

  • Your legal spouse or domestic partner
  • Your children up to age 26 and those unable to care for themselves due to disability

Your dependents must be verified. Please see the list of acceptable documents here.

When Do My Benefits Begin?

You (and your eligible dependents, in many cases!) are eligible for the following benefits effective as follows:

*Benefit available only to employees; dependents are not eligible for this coverage.
**This benefit is fully paid for by Amy’s and is not available to dependents.

Enrolling & Making Changes

During enrollment you can select a combination of benefits that fits you and your family. The coverage you choose is in effect through the end of the plan year (December 31). 

You have three opportunities to enroll in or make changes to your benefits:

Within 30 Days of Eligibility

If you’re newly eligible, this is your first chance to choose coverage!

During Annual Open Enrollment

The Annual Open Enrollment period happens once a year, usually in October. The coverage you elect during Annual Open Enrollment begins January 1.

Within 30 Days of a Life Event

Had a major life change? Your benefit coverage can change, too. Update your benefits within 30 days of the life event.

Mid-Year Changes

Got Married? Had a Baby? Lost Other Coverage?

These are qualifying life events (QLEs), and they may allow you to update your benefits outside of open enrollment. But act fast! You have 30 days from the event to make changes. Log in to UKG Benefits Administration to request a change.

Here’s the list of QLEs:

  • Marriage, divorce, or legal separation
  • Birth or adoption of a child
  • Death of a dependent
  • Loss or gain of other health coverage for you and/or dependents
  • Change in employment status
  • Change in Medicaid/Medicare eligibility for you or a dependent
  • Receipt of a Qualified Medical Child Support Order 

Enroll Now

Your enrollment period is an important time to review your benefit options and choose the best coverage for you and your family. You can enroll for benefits within 30 days of your eligibility date or during the annual Open Enrollment period. No enrollments or changes will be allowed after your initial enrollment period or after the annual Open Enrollment period ends, unless you experience a QLE.
